Self-pay at this time. Insurance credentialing is in progress. We'll list each carrier here only once it's active and we can actually bill it, so nothing on this page turns out to be wrong when you call.
Superbills. If you have out-of-network benefits, we can provide a superbill you can submit to your insurer for possible reimbursement. Whether they reimburse, and how much, is between you and them.
Cancellations. Please give at least 24 hours' notice if you need to cancel or move an appointment. Later than that, or a missed appointment, is charged $100.
Your right to an estimate. Because we are self-pay, you are entitled to a Good Faith Estimate of your expected costs before you are seen. Read about that here.
What's included
The fee is the visit. Nothing is bolted on afterwards.
The appointment itself
A full 60 minutes for a first visit, 30 or 45 for follow-ups. Not a rushed slot with a longer one billed separately.
Your plan and prescriptions
Sending prescriptions to your pharmacy and adjusting them between visits when it's clinically appropriate.
Billed separately by others
Medications, laboratory work, and care from any other provider are billed by them, not by us.
